SYMPTOMSWhen you customize the Insert menu in Microsoft Office Word 2003, some of the custom menu items may be lost, rearranged, or repeated on the Insert menu.CAUSEThis problem may occur when you add custom menu items to the Insert menu, and then you make changes to those custom menu items.RESOLUTIONThis problem was first fixed in a hotfix that is now contained in a service pack.
