COM+ 1.0 Cannot Install DLL Modules with COM Activities in DllMain (811373)

SYMPTOMS When a dynamic-link library (DLL) module has CoCreateInstance activities in its
DllMain function, COM+ cannot install the DLL, and the failure returns HRESULT 0x800401F0(-2147221008) - CoInitialize has not been
