SYMPTOMS

The BeforeAttachmentSave event does not run when changes made to an attachment are saved.

CAUSE

The event runs when the item is saved because this is when Outlook saves the attachment in the item. It does not run when the attachment itself is saved as a temporary file on disk.

RESOLUTION

This is how the event is designed to work.
