Even though the "Smooth edges of screen fonts" option is enabled, smoothing
is disabled in an application that uses the ETO_GLYPHINDEX flag when
calling ExtTextOut().

Applications that take advantage of OpenType Layout for rich typography
that access glyphs directly from a font (such as Adobe PageMaker), or
applications that access characters not in Unicode, may see grayscale with
Glyph index text disabled.
