SQL Setup Failed: OpenDBConnectionFailed (MCIS) (164040)

When you install Microsoft Internet Address Book Server with SQL Database
Configuration, you may receive the following error:
SQL Setup Failed.: OpenDBConnectionFailed
The only option is to click OK. Microsoft Internet Address Book Server
setup then closes.
CAUSE
The MSSQLServer service has not been started or the SQL Server has not
been registered.
RESOLUTION
Start the MSSQLServer service. Launch SQL Enterprise Manager and register
the SQL Server by entering the server name and the account information.
If you are prompted with the following dialog box, click Yes.
Unable to Connect to Server
(reason: (DB-Library) Unable to connect: SQL Server is unavailable or
does not exist Specified SQL server not found). Register anyway?
If you receive the following error, make sure the MSSQLServer service
has been started.
A connection could not be established to [MACHINE]

MORE INFORMATION
You must uninstall Microsoft Internet Address Book Server before
attempting to reinstall. Use Add/Remove Programs in Control Panel, follow
the instructions for proper de-installation, and then reinstall Microsoft
Internet Address Book Server by running Setup.exe from the appropriate
directory.
