This patch corrects several known problems with salvage:
  - Fixes two infinite loops that could make salvage run forever.

This patch fixes a problem with the /sbin/advfs/salvage utility which
could cause the utility to core dump when attempting to salvage data
from an AdvFS file domain. In addition, there has been a general
performance improvement made so that salvage should run on average
about 20% faster.
