The S3 Trio64V+ graphics card (PB2GA-JC or PB2GA-JD) was not being uniquely
identified by the driver at startup.  The identification string that the
driver prints at startup now includes the "V+" suffix to distinguish between
the S3 Trio64V+ card and the original S3 Trio64 card.
