OL2002: Error Message: Outlook Has Caused an Invalid Page Fault in Module Da0350.dll (810935)

SYMPTOMS

When you try to send a mail message in Outlook 2002, you may receive an error message similar to the following:
Outlook has caused an invalid page fault in module da0350.dll
.

CAUSE

This issue may be caused by MegaPhone, a screen-based telephony program from The MegaPhone Company. The MegaPhone Address Book is not compatible with Outlook 2002.

RESOLUTION

To resolve this issue, remove the MegaPhone Address Book from Outlook. To do this, follow these steps:
  1. Start Outlook, click Tools, and then click E-mail Accounts.
  2. Click View or change existing directories or address books, and then click Next.
  3. Click MegaPhone Address Book, click Remove, and then click Finish.
