SYMPTOMS

If you unplug a hard-drive that is attached to a storage controller and then click Refresh in Device Manager, the computer may stop responding. This may occur if the device driver for the controller is a SCSI miniport driver.

CAUSE

This behavior can occur on Windows Millennium Edition because of a conflict between the I/O Supervisor (Ios.vxd) and the disk "SCSI'izer" (Diskvsd.vxd, the program that builds SCSI Request Blocks for I/O requests that are sent to SCSI devices).

Third-party developers frequently create SCSI miniport drivers for non-SCSI storage controllers, such as IDE controllers, for these operating systems. Some of these buses support the concept of surprise device removal, such as the Serial ATA specification.
