FIX: "Operation Failed" error message when you try to paste a file into a message in Outlook 2002, or the file is not attached when you try to attach it in Outlook 2002 (308548)

SYMPTOMS

When you attempt to paste a file into a message that you are composing, you may receive the following error message:
Operation Failed
This occurs with an extension that implements IExchExtAttachedFileEvents::OnReadPattFromSzFile. Alternatively, if you attempt to attach a file by using the Insert File command, you do not receive an error message, but the file is not inserted.

CAUSE

The return of S_OK from a client extension's implementation of IExchExtAttachedFileEvents::OnReadPattFromSzFile causes Microsoft Outlook to abort the attachments.

Outlook 2002 changed the way Outlook reacted to return values from OnReadPattFromSzFile. In earlier versions of Outlook, if the extension returned S_OK from OnReadPattFromSzFile, Outlook continued processing of the attachments and the file actually got attached. In Outlook 2002, the return of S_OK causes Outlook to throw an E_ABORT return code, which aborts the attachments process.

RESOLUTION

To resolve this problem, obtain the latest service pack for Microsoft Office XP. For additional information, click the following article number to view the article in the Microsoft Knowledge Base:

STATUS

Microsoft has confirmed that this is a problem in the Microsoft products that are listed at the beginning of this article. This problem was first corrected in Microsoft Office XP Service Pack 1.
