The AlphaServer GS140/GS60 will crash with a simple lock timout or TB
shoot ack timeout panic. After the dump is written, the message
"CPU # not halted" will print on the console for two processors on the
same module (for example CPUs 6 & 7 on the module in slot/node 3. This
problem occures on large configurations with two IO Port modules and a
large number of SCSI buses.
